NEW YORK (AP) — A report says many factors led to an autistic teenager walking out of his New York City school to his death — including a safety agent distracted by a hug and a principal who didn't want to alarm children with a lockdown.

The report released Thursday also says 14-year-old Avonte Oquendo's teacher failed to pass on a warning from his mother that he was prone to wandering.

The mother, Vanessa Fontaine, says "someone has to pay" for her son's Oct. 4 disappearance from school. His remains were found in the East River.

The city schools investigator's report says the school safety agent at the front desk was distracted when a student hugged her. It says the principal refused to institute a soft lockdown because he didn't want to alarm students.
